We are CARF Accredited and offer services to those living within Adams, Allen, Blackford, DeKalb, Delaware, Huntington, Jay, Kosciusko, LaGrange, Madison, Noble, Steuben, Wabash, Wells and Whitley counties.

The Ft. Wayne operation prides itself on having a longstanding, dedicated leadership team with over 100 combined years of experience averaging 17 year tenure per person with the company. It is a very diverse and welcoming operation with excellent nursing and dedicated management personnel.
