Jennifer Jeffrey: Wish I could give a 0 star. The staff is horrible! Father was placed into this facility after a full hip replacement. First night my father used the call light up to 30 min and no one came, my father attempted to go to the bathroom after being ignored and fell. When in he bathroom he pulled the emergency cord but no one came for 30 min. They then slapped an adult diaper on him and left him in bed incoherent for 18 hours. He was severely dehydrated and blood sugar fell to 35. EMS had to come get him and was in ICU for 3 days . Never gave appropriate meds and was over medicated. No one there knew what meds to give and every time I would go find a nurse they acted like I was a bother, please save your loved ones by not putting them in this horrible facility.

Breanna Adcox: I worked for this facility. I will say it was very clean and decent staff to patient ratios compared to other facilities. However, management is not efficient about a lot of things. It is not a good work environment at all, it got very toxic in the time I worked there. Went from being a great facility to one I dreaded going to work every day. Then they proceeded to fire me, not even following their own protocol. They also mess with the staff's paychecks enough to cause issues. It has happened a few times to myself, and still happening from what I heard. For families, the care is great. For staffing, if you want to avoid unnecessary stress, I would avoid working here.

Daniel Spieth: I have called and left a message to schedule a tour and get information. Separately, I completed a tour request form to get information too. Both of these requests were done over one week ago and still no one has contacted me. This is the only facility that has failed to call me back. If they cannot do simple things like return phone calls or follow up on web requests, I would not trust them to take care of a family member.

Nancy Riddle: I was working with the case manager at Lutheran General and was told my mom couldn’t go into a nursing facility because of her aggressive state she was in. I thought it was hospital dementia. No one at Lutheran would say what it was exactly, but we were told no nursing home would take her. I was nervous and scared. The case manager said Stillwater would. The team was professional from start to finish. I first met with the intake nurse who explained everything to me. She said that the head of Stillwater’s geriatric doctor visited my mom and reviewed her charts…she knew immediately that my mom had sepsis delirium-no other doctor in the Lutheran General Hospital in Ft. Wayne knew this when my mom was in ICU! Stillwater took my mom immediately from Lutheran via ambulance and I followed. They had a room ready. I never had to handle any bills-there was just an intake form about my mom’s likes and dislikes-favorite music, foods, etc. They had a care package of food and water ready in the room for me, too. I met with the social worker and also the head nurse, Wava. They were awesome. They answered all my questions, including if my mom could stay on extra days, which she could…for $250/day, which is a good rate! We were initially told a maximum of one week, so that’s my only criticism. This is an inpatient hospice, which I learned is different and required for cases like my mom, because she would have hurt herself or others because of the delirium. They took excellent care of my mom. They were all so kind. My mom was so clean and smelled so nice from her baths and her clean sheets. They had a fidget blanket which told me they understood their patients. She was kept comfortable on her pain meds.The grounds are gorgeous. Oh, and I accidentally left my laptop there and was too exhausted to go back and get it. They kept it in safe-keeping for me…it wasn’t stolen so I was glad I could trust them with that, but I don’t recommend leaving valuables on your loved one or in the room. This is a non-profit, which I loved, too.

Allison Liddy: If peace and comfort are what you’re looking for in your loved ones final days then keep looking.Following a 2.5 year battle with cancer my 7 year old daughter was placed on hospice. The job that Stillwater had with our family was providing comfort and peace at home for her final days, the only thing I could ask for and something I was under the impression was their purpose. The only pain medication provided was morphine and when that wasn’t working they recommended the hospital. So, they recommended we do the one thing we hired them to avoid. I’d asked for other options for days before it got “really bad”. Then, as Saturday rolled around the fact that it was a weekend was an excuse for their neglect.Her battle brought a lot of traumatizing experiences, but the utmost traumatizing moments experienced were at the hands of Stillwater.The generic response you’ll reply with about grieving and HIPAA is demeaning & a poor response to the lack of care provided.

Janet Workman: I do not have my husband at the home but he has been on Visiting Nurse/Stillwater Hospice for 1 1/2 years and I cannot praise the care he has been given, the deep concern for him and the family is amazing, I have not found one employee that I can complain about. I recommend it very highly

Lindon Robertson: Our experience with Stillwater was excellent. Caring, compassionate, and responsive. We felt they were there for a very important purpose and knew it, and each person helped to fulfill it.

Stacys Ratcliffe: A great experience. Nurses answer any question. They are very patient with family. They are very caring to the patient. They understand that the family is going through a process that has many stages. They are special people for choosing to work there.

Bill O'Neill: Our family had a wonderful, kind, caring experience with the staff at visiting nurse and hospice. We felt very fortunate and grateful that our family member could spend his last days and hours in their care.

Latika La-Shae AndThenWhat?: I can't say enough good things about Visiting Nurse hospice care. They made my grandmother's transition wonderful. Every time we needed something, Visiting Nurse was delivering it almost immediately (within an hour). I can say so much about them but I don't have the space. If my family ever needs hospice care again, we will definitely be calling Visiting Nurse.

I wouldn't come back to put my dog down here.If you feel like the nurses are being nasty PLEASE secretly record the conversation with your smart phone as evidence and send it up the ladder.I dont think they care though either - as you can see they use the same copy & paste reply that its false information and want to prosecute (for telling the truth). Which is why its SO important to record all conversations with nasty nurses on your smart phone.

Liz: This place is awful!! My father in law is in their care now..he hads huge bed sores that smell like rotten flesh!,its just awful!! They just keep him drugged up and say there is nothing to do to help heal the sores.

Diana Arnold: This is where my mom spent her final weeks. I can't say enough about this place and the staff. This was back in 2005, but I still think about this place a lot. Having a cutting garden to get fresh flowers for her each morning was a huge deal! All the way around, it's an awesome place to spend the worst part of my life.

My Daughter spent the last nine days of her life here.The staff did everything in their power to make her feel comfortable. I could use all the other words like compassionate, caring, respect, dignity and the staff was all of these, but most important they treated us like FAMILY.Yes I would highly recommend.

Cole Randol: It was a difficult choice to put my mom in hospice, but I am so thankful we did and chose VNH. We spent 11 days there with mom. I can not praise them enough. My mom had excellent care, not one time did we ever have any issues. Every staff member went above and beyond in everything they did to make my mom and sisters feel comfort and peace. They are the most compassionate, caring people ever! They treated us like family, and for that I am truly grateful.

Carolyn L: My brother Kerry Ridge spent his last days here. Every person I talked with, every person who helped him and myself were some of the most compassionate, loving people I have ever met. They treated him and his family like we were family, with respect and dignity. I would recommend Visiting Nurse in Ft. Wayne to everyone.

Heather Desotell: My mom passed away here 4 weeks ago from the time we found out she had cancer until the time she passed away was only 6 weeks she was only in the facility for 1 1/2 days she was fine going in other than in a lot of pain so she was there to get the pain under control we were supposed to take her home the following morning. It happened extremely fast after we took her there. Since this time trying to deal with the grieving I have done some research and have found that they had my mom on a high dose of methedone and they were also giving her ativan for anxiety and now I see that those two medicines together are lethal. So yes I feel they killed my mom!!

Akezia Bowen: They provide very poor training, they do not train properly and the communication is no good. This is not a company I would not work for again nor place any family member in to be cared for.

Barbara Renz: Staff is unfailingly helpful and kind as well as efficient and professional.Facility is very clean: nice restrooms are much appreciated. In many, many years of visiting my mother and then my sister, I have never been offended by any situation.The Chapel had a very nice update in the past decade or so. The rest of the facility is well-kept, (if a bit unimaginative.)Most important though, my sister is 100% in the Courtyard apartments, and really considers SAH to be her cherished home and family, and daily Mass is her joy.

Del Campbell: Visited my uncle and the entire staff was wonderful. Took him to the coffee bar and he lobbed the coffee and brownie and sandwich! Nice experience. There was an incredibly nice sweet nurse or orderly on the 3ed floor who stopped while my uncle and I were looking at some of the old pics of fort wayne buildings. She stopped to join us, and that was really nice. I regret I didnt ask her name but will next time. She was very sweet and had just a really fun look about her! Also, I visit regularly, as does my other uncle and Mom. We have never had a bad experience and my uncle who is there consistently says this is a nice place. Someone in my family is there at least once per week and always nice.

Chad Parrish: I came to the facility as a vendor. I was greeted by the staff and assisted with the screening process. Very easy and the receptionist was super helpful. The building was very clean and clutter free. The residents appeared well kept and clean. I observed several staff members interacting with the residents. I was there in a hospice capacity. The staff was knowledgeable and friendly. Good visit from top to bottom.

Joseph Millhouse: My mother in law was in this facility for rehab. Unfortunately, she broke her arm at another facility (unreported) and had to have total shoulder replacement. She was in the rehab side beginning in nov 2020 but when covid broke out due to their lack of care, she was moved to the residential side. Soon after, she got covid from an infected roommate. While talking to social worker, my mom requested to go home. Social worker was condescending toward her and put roadblocks into her mind to not go home. They did not have her best interests and clearly, did not have to keep her there. They threatened her by saying if she left , she may have to pay for her entire stay or down the road, may not be able to get back in again. After getting covid and being quarantined, she ended up getting a second break, this time a spiral break in her humerus. Sadly, my wonderful mother in law passed from covid pneumonia 12/19/2020. None of this should have happened. She wanted to be home and they did not listen to her or care for her well being. Never take your loved one here.

Debra McNamara: My mother spent her final weeks at St Anne's. Everyone there was so kind and helpful. They were quick to respond when my Mom needed anything. They were very gentle when moving her and always told her what they were going to do before they did it. They checked on her like clockwork. They also checked on us to see if we needed any support. They even brought a snack cart into the room as we were there everyday all day. It gave us great comfort to know she was in the best place with the best care. When we left at night to go home, we knew she was in good hands. They made her very comfortable in her final days and that gave us great peace and comfort.

Dave B: The best place for your loved one. The staff are wonderful, caring, respectful and loving. Having my mother there is comforting and gives her and us peace. Cannot say enough about Saint Anne's.

Ali Bauma: An absolutely wonderful place. They actually put the patients first and make sure everything is neat and organized. The patients are happy and there are so many things they can do. On nice days they can go outside to one of the many courtyards that they have. They put pictures of patients on the wall, and there are always decorations up so there is plenty to look at. I recommend this place to everybody. This is so much better than any other place. You will not be disappointed, I promise.

D G: Admin staff is more than helpful and very attentive to family needs. Even if family Dynamics are a bit odd, they are more than willing to try to accommodate and understand everything that is going on. They definitely make you feel human when trying to juggle life in general.

David Oviatt: Went in for an interview that they asked for. Had 2 emails that stated they looked at my resume. After I got there was told I didn't have enough experience and basically I had to tell them that I wanted to still do the interview just to ask them why I had been asked out. Was not given the same experience as the other people you requested to interview. Here is a small idea if you said you looked at a resume maybe you should, instead of having someone come out and waste their time and money

April Jones: Life Care Center is a great atmosphere to be in. Their administrative team really makes sure their residents are cared for. I have been here several times, it is clean and welcoming.

Suku-Chan (LeiLei): I see they got people from Management on here lying. I didn't come on here to slander the place even though I have seen and heard enough for a state to get involved. But to sum it all up a change needs to happen. Your staff will only be as good as management it's only going to get worse over there. I was there 4 years. And I appreciate the experience I gained during my time there but corporate needs to throw the whole management team away and start over it's toxic. And the only people who are suffering are the residents.

April Christman: They really care about their residents! Management has the best interest of the residents. The activity director is amazing and makes sure the residents enjoy their days! She is great at putting a smile on all of their faces!

TurboCoon: Grandpa was rehabilitated here for his knee and I must say, best and most up-kept, nursing and rehabilitation center I’ve seen. I’ve had my fair share of grandparents in these types of places and nothing has came even remotely close to the performance, Cleanliness, and nurturing of Summit City. The staff and E.D go above and beyond for their guest and families.

Destiney Bead: I don’t leave reviews, but I must after learning my father in law is walking better then he ever has. He rehabbed for 2 months at Summit City following total knee replacement. At that, it was during a pandemic. The team is so friendly, so nice, understanding and selfless. The ED is very helpful and knowledgeable when it comes to all the pieces for success. Great surveys, great hospitality, great care, period. Did I mention that the ED is the G.O.A.T of LTC. 🤷🏼‍♀️ just saying. Team summit rocks!!!

Vickey: Nice clean building. My husband was rehabilitated with his total knee replacement. Physical therapy department was very thorough et patient. Different department managers visited my husband to meet his needs. Also the care was timely et efficient.The rooms were mopped et cleaned every day . Snacks were offered every day et news papers with daily activities provided . Food was very proportionate et tasty. A very nice et pleasant rehabilitation stay. Thank you ! Et especially to the Administrator et Staff .

Might I add the new management team is coming together nicely. Even with staffing challenges everywhere, this team finds a way to get it done! My family and I were very pleased with the service that my mom received at the facility. We thank you to all of the administrative staff, CNA, nurses, and others who helped my mom on a daily basis. I am confident to recommend the Summit City Nursing & Rehab to everyone who has the need of care for their loved one. Again, we appreciate you all very much for all you do for my mom.
