Rochelle Tollinchi-torres: It takes 15-20 minutes on any given day for anyone to answer the call button being on. The room always needs attention! Floor is dirty, trash from employees supplies on floor, trash not always emptied.
Janet George: My father-in-law had three falls in three weeks at this place. Two landed him in the hospital with head injuries. Items repeatedly missing from his room. Upon moving him out some of his personal belongings had been stolen.
CanterburyNursingCenter facility: Our sister has been at Canterbury about six months. We are extremely pleased with her treatment and with our interactions with the staff. Any questions are answered promptly. The staff is courteous and makes us feel welcome. Most importantly, our sister is happy here. Our family strongly believes that the staff at Canterbury has saved our sister's life, not once, but twice because of their prompt action and we are so grateful.,

morgan jones: if I could give this place no stars, I would. most residents have problems like not getting their medicine on time, not being woken up for meals, frequently falling and much more. many of the staff seems to not care about the people there, they even go to the point of talking trash about the residents in the hallways right in front of them. the nurses and CNAs will also purposely ignore call lights.

Jacquie Downey: The clinical team and other staff are great to work alongside. I see the care and compassion they have for all the residents. I love that we are able to have both a male and female Memory Care wing in addition to offering Long Term care, Rehab, respite and hospice.

Dennis Bushur: My mother had a massive stroke, was immobile, non-verbal and could only communicate by facial expressions. We placed her at Canterbury since it was near family (my brother and I both live 2 hours away). Her room was always dirty, had lots of bugs and had residents with dementia wandering in and out of her room since they did not have a locked down unit. The final straw was when transportation dropped her off at her doctor's office at Parkview North without checking her in at registration. My brother was running late coming from Michigan and when he got to the facility, they said no one had checked her in or seen her. He knew she was there because he called the facility and they said she was "dropped off." He and security searched the building and found her in her wheelchair in the corner of the waiting room at the hospital entrance, not the professional office building where her appointment was, with no identifying papers or a sitter present. The parking attendants had found her and were trying to communicate with her while they assumed a family member was getting a car to pick her up (she had been there at least an hour). When we contacted Canterbury, they did not have a policy of a safe sitter staying with their client while waiting for a doctor's appointment if family could not be there. They told us the driver would occasionally stay there but if they had other drop offs, they were not able to do so. I was very discouraged and disgusted because my mother had no voice and they did not keep her safe enough to prevent this from happening. I highly recommend another facility since the quality of care and caring demeanor was sorely lacking.

Vanilla Sunrise: I have been an aide for 10 years and this is the worst nursing home I have worked in. The staff does the least amount possible. I have come in on my shift and had residents that were soaked and full of poop and some were dried on then like they hadn't been changed all day. The staff is always short and we have to do double halls and less care for each resident. It was so stressful that we went through 3 schedulers in the last 2 months the newest however tells us that she doesn't care about her job because she gets paid regardless is we are staffed or not. The DON has been left several notes and has done nothing to fix any concerns. Staff calls off all the time and do no call no shows and still have jobs. It is overall the worst place I have worked and am glad to be going somewhere else it is so stressful my blood pressure was high for weeks I could not handle the stress any more. So if you ask me I would not work here and I would not put my worst enemy here as a resident.
Selina Shaw: My great grand mother is currently there and the staff provide her with excellent care. The other residents are friendly to her and most of them call her Grandma. She loves it there and IM very glad we made the right choice.

Jenniffer Price: I am blessed to work as an NP at a facility with a management team that truly cares about their residents. They serve a specialized population and do it with care and compassion on a daily basis.
David Johnson: Dogs deserve better treatment. My grandmother was sent here for rehabilitation and the staff couldn't lift a finger to help her. The room she shared was at an insane temperature. They made her sit for hours on a portable ventilator. We asked for a regular ventilator and none of them worked (I thought this was a medical facility, so why is all this equipment badly maintained?). This place is a crime against humanity
tasha finley: I can't understand how this place has almost 4 stars, when I stayed wet for days, majority of the staff don't care anything about their patients. People stole from me , barley bathe me, didn't even want me to go to an hospital; although, I had serve chest pains and everyone was getting sick in this nursing home. I called 911 my self! that's when the staff decided to take me to Lutheran hospital. I ended up in ICU. I had pneumonia! I watch a cancer patient of theirs fall right in front of the staffs. The only reason why I gave it one star because they replace my partials and the activity; other than this, It's been about 4 years since I been there and I have not been the same. They were very interested in money. I watch when the state was coming out they start doing their job then and only then. I will not send my dog to live here.
Martin Adair Hibbert: Everything about the place is awesome!.....the staff is awesome, the rooms are clean, the food is great and everything about this place is up to standards. When family or freinds visited the staff was pleasant and respectful. They provide stuff to do activities with residents here as well. Staff is always happy to help you without any complaining what so ever. I've had an excellant experiance and recommend anyone that the stay here will be pleasant. Susan Longbottom: Unskilled facility. Very dirty. Staff does not follow through with tests needed on patient basically they tell you what they think you want to hear. My dad was diagnosed with cellulitis at Parkview hospital and sent for rehab and this facility didnt do anything to help him and said he was fine. Didnt have cellulitis when his legs and feet where so swelled and had sores that bleed. They ignore requests for treatment. Was supposed to get a psych evaluation and did not follow through. Nurses are very short tempered and not helpful.

Naing Htoo: The experience my grandma had from here was excellent, the nurses were all so kind to her and had a lot of a patience. They had the best food for her when she was unable to eat the food there. They set up the perfect diet for her, this place was nice and clean. A inviting environment to be in.

They are extremely rude at the front desk when I went to drop items off for Leona Resendez. They told me put in on thr floor in a very rude tone. It was the HHCA lady behind the desk. This is the 2nd time they were very very rude. I’m a caretaker and several residents have called me in tears regarding the treatment. I would not send anybody there!!! I may add they didn’t say can I help you? Nothing. They seemed bothered by my presence. If they treat me like that how are they treating the residents? They had no idea if I was there to ask about the facility or what. In fact I did have someone that’s needing rehab but after the 2nd time of being treated with such disrespect I was done!!! I’d like a response as to why I was treated like that. Also I was dressed in my scrubs with my badge on and got no respect. Again they had no idea why I was there. I was also trying to inquire about availability. I didn’t get that far due to the unprofessional behavior!!!

Jessica Mullins: I stayed at Glenbrook for 2 months to finish a round of IV antibiotics. The place is clean,the food is decent and the staff is great. It is a relaxed atmosphere filled with laughter. The activities staff has created a calendar full of things to participate in if you choose to do so. I was happy to go home but part of me hated to leave.

Adam Thomas: Pretty horrible experience for my mother so far. The meals are never hot meals as I have been there many times when they are brought in. I wouldn't really even consider it a meal as it's usually a cold sandwich with chips, no real regard for my mother needing a diabetic diet either so we have to have groceries delivered quite often. Something we shouldn't have to do after being charged almost 10k a month! We also recently learned from the nurses that my mother has not been receiving regular showers for a few months now. Very neglectful facility overall! Definitely would not recommend to anyone!

Was very impressed when the director, Holly came by and introduced herself. Keep up the great work. Your kindness and caring hearts will pay you all back ten fold. David Oviatt: Went in for an interview that they asked for. Had 2 emails that stated they looked at my resume. After I got there was told I didn't have enough experience and basically I had to tell them that I wanted to still do the interview just to ask them why I had been asked out. Was not given the same experience as the other people you requested to interview. Here is a small idea if you said you looked at a resume maybe you should, instead of having someone come out and waste their time and money

But to sum it all up a change needs to happen. Your staff will only be as good as management it's only going to get worse over there. I was there 4 years. And I appreciate the experience I gained during my time there but corporate needs to throw the whole management team away and start over it's toxic. And the only people who are suffering are the residents.

Sandy Armstrong: If you want your loved one to get COVID and be ignored bring them here! be prepared to fight every step of the way for them to receive good care.
D Hilton: My father was there recently for only 5 days and most of the staff are not accountable with their duties. Their staff also, do not show any dignity towards their clients. My father fell twice and only one fall was reported to me. Moved my father on day 6, I regret waiting that long. I strongly urge you look at other facilities for care for your loved one.

Sheila Worthington: My father was on hospice there. I went to see him and staff had vague answers and little follow up to questions. I questioned if anyone was taking care of him but hospice staff. I tried to call the facility to get an update several times and no one answered the phone a day later. The last time it rang was 20 times. I was in tears and called the hospice provider.

I checked to make sure bed pad that I brought from her home (they don’t use any) was placed in proper place and was shocked to see the bed and sheets were soaked with urine. Someone made the bed without changing the wet sheets!!!! Everyday it’s something.

Heritage Park: what do users think?
Aliyah Byrd: They took great care of my grandmother while she was there! Awesome therapy program and the nurse that cared for her was amazing. The social worker helped my family through the whole process also. Very grateful for them! Could use more staff but that’s the case everywhere.
Tony Miller: I've only been here for a few days. I am staying her for speech/physical & occupational therapy. They are great with keeping up with my meds.but housekeeping isn't doing well. I don't get drinks with my meals.

Beth Nicholson: My mother was admitted to this facility and in the first three days of her stay they waited too long to take her to the bathroom and she didn't make it. She turned on the call light to get help and it was on for thirty minutes before anyone came. Last but not least she had turned on her call light for help to get into bed and get comfortable for the night and next thing she knew her night nurse found her sitting up in the chair covered up with one of her hospital gowns and it was three a.m. The call light was still on and her feet were so swollen she couldn't get her shoes on. And she is only there for short term rehab stay!!!!

Charles Wood: my dad went in for rehab and right before he was to be released he received a covid test, and it came back positive. what a coincidence. he was then moved to another room and asked several times to be moved to another cause the patient in the next room argued with someone on the phone for several days and he wasnt able to have peace and quiet. Those requests went ignored. He is not showing any symptoms for covid so he can easily be sent home to self quarantine. The patients there are nothing but dollar signs to the facility. He came there straight from the hospital so how is it he tests "positive" while in there just before he is to be released to go home???????

Summit City Nursing and Rehabilitation: what do users think?
Paris Coon: I can’t say enough good things about this rehab center. A friend’s father had a stay there after contracting covid, which led to his having a stroke. When he arrived, he couldn’t even feed himself. The nursing, therapy, and administrative departments never gave up on him, and provided excellent care throughout his long recovery. It took months, but due to their diligent efforts, my friend’s dad regained his independence.We can’t thank the team at Summit City enough for bringing him back to us.
TurboCoon: Grandpa was rehabilitated here for his knee and I must say, best and most up-kept, nursing and rehabilitation center I’ve seen. I’ve had my fair share of grandparents in these types of places and nothing has came even remotely close to the performance, Cleanliness, and nurturing of Summit City. The staff and E.D go above and beyond for their guest and families.
Destiney Bead: I don’t leave reviews, but I must after learning my father in law is walking better then he ever has. He rehabbed for 2 months at Summit City following total knee replacement. At that, it was during a pandemic. The team is so friendly, so nice, understanding and selfless. The ED is very helpful and knowledgeable when it comes to all the pieces for success. Great surveys, great hospitality, great care, period. Did I mention that the ED is the G.O.A.T of LTC. 🤷🏼‍♀️ just saying. Team summit rocks!!!
Vickey: Nice clean building. My husband was rehabilitated with his total knee replacement. Physical therapy department was very thorough et patient. Different department managers visited my husband to meet his needs. Also the care was timely et efficient.The rooms were mopped et cleaned every day . Snacks were offered every day et news papers with daily activities provided . Food was very proportionate et tasty. A very nice et pleasant rehabilitation stay. Thank you ! Et especially to the Administrator et Staff .

Akezia Bowen: They provide very poor training, they do not train properly and the communication is no good. This is not a company I would not work for again nor place any family member in to be cared for.
Barbara Renz: Staff is unfailingly helpful and kind as well as efficient and professional.Facility is very clean: nice restrooms are much appreciated. In many, many years of visiting my mother and then my sister, I have never been offended by any situation.The Chapel had a very nice update in the past decade or so. The rest of the facility is well-kept, (if a bit unimaginative.)Most important though, my sister is 100% in the Courtyard apartments, and really considers SAH to be her cherished home and family, and daily Mass is her joy.

Del Campbell: Visited my uncle and the entire staff was wonderful. Took him to the coffee bar and he lobbed the coffee and brownie and sandwich! Nice experience. There was an incredibly nice sweet nurse or orderly on the 3ed floor who stopped while my uncle and I were looking at some of the old pics of fort wayne buildings. She stopped to join us, and that was really nice. I regret I didnt ask her name but will next time. She was very sweet and had just a really fun look about her! Also, I visit regularly, as does my other uncle and Mom. We have never had a bad experience and my uncle who is there consistently says this is a nice place. Someone in my family is there at least once per week and always nice.
Chad Parrish: I came to the facility as a vendor. I was greeted by the staff and assisted with the screening process. Very easy and the receptionist was super helpful. The building was very clean and clutter free. The residents appeared well kept and clean. I observed several staff members interacting with the residents. I was there in a hospice capacity. The staff was knowledgeable and friendly. Good visit from top to bottom.
Joseph Millhouse: My mother in law was in this facility for rehab. Unfortunately, she broke her arm at another facility (unreported) and had to have total shoulder replacement. She was in the rehab side beginning in nov 2020 but when covid broke out due to their lack of care, she was moved to the residential side. Soon after, she got covid from an infected roommate. While talking to social worker, my mom requested to go home. Social worker was condescending toward her and put roadblocks into her mind to not go home. They did not have her best interests and clearly, did not have to keep her there. They threatened her by saying if she left , she may have to pay for her entire stay or down the road, may not be able to get back in again. After getting covid and being quarantined, she ended up getting a second break, this time a spiral break in her humerus. Sadly, my wonderful mother in law passed from covid pneumonia 12/19/2020. None of this should have happened. She wanted to be home and they did not listen to her or care for her well being. Never take your loved one here.

Debra McNamara: My mother spent her final weeks at St Anne's. Everyone there was so kind and helpful. They were quick to respond when my Mom needed anything. They were very gentle when moving her and always told her what they were going to do before they did it. They checked on her like clockwork. They also checked on us to see if we needed any support. They even brought a snack cart into the room as we were there everyday all day. It gave us great comfort to know she was in the best place with the best care. When we left at night to go home, we knew she was in good hands. They made her very comfortable in her final days and that gave us great peace and comfort.
Dave B: The best place for your loved one. The staff are wonderful, caring, respectful and loving. Having my mother there is comforting and gives her and us peace. Cannot say enough about Saint Anne's.
Ali Bauma: An absolutely wonderful place. They actually put the patients first and make sure everything is neat and organized. The patients are happy and there are so many things they can do. On nice days they can go outside to one of the many courtyards that they have. They put pictures of patients on the wall, and there are always decorations up so there is plenty to look at. I recommend this place to everybody. This is so much better than any other place. You will not be disappointed, I promise.

Neil Schafer: Unfortunately what previous reviews state seems true on some matters. Unfortunately it does smell strongly of urine. And as well if u have a lived one there u really need to monitor their food as it's brought in to see the quality. Tonight a folded slim piece of ham on a blot of cheese on a small bun bisquite so hard the bread was I could not bite it. As well the onion rings were rock hard. I witnessed one elderly person out of frustration put it down and bow her head. It was all she had to eat and could not eat it. I understand the short staffness but we aren't short yet on human compassion I hope.
C.V. Bryant: Unprofessional on many levels. Staff or an administrator has their young kids running around at night. Special dietary needs sent from hospitals/ doctors are not met. It's hours before they check on patients, call lights are on for hours. Patients that have incontinence and or bowl issues are left in the soiled garments for hours. Some staff are hostile towards the patients family members if they question the care that's being provided. They also don't always send all of a patients personal belongings and then play games or are extremely rude and beligerent when a family member calls and inquires. Absolutely Ridiculous facility.

Coventry Meadows: what do users think?
Dawn Staton: I’ve been dreading this review since my dad passed over a year ago. My dad first came to CM after a heart bypass in 2011, and stayed 100 days for rehab. I visited him every day for those 100 days. It was an excellent facility back then. As my dad aged and began having falls, along with Alzheimer’s, I knew it was time to place him on a permanent basis. I knew CM was the place for us. They started out great. My dad was there for some time when the director told me they wanted to move him to the Alzheimer’s hall. The level of care fell significantly on that hall. (I found out they will pick people randomly and put them in the memory care unit because they are able to charge a ton more.) The problem is most people do not visit their loved ones often. I visited my dad almost daily for three years. So I had a unique position to see and hear a lot of things most others did not. After Covid the care went down even further. I wish I could write and tell just how awful it became. I have both pictures and videos of it all still on my phone. In the end, they called the police to escort my 79 year old, wheelchair bound dad, from the facility. They didn’t call me until my dad was gone. They wanted him out because they knew I had a lot of dirt on them. When I arrived at Parkview, I was informed by medical staff there that they refer to Coventry Meadows as Coventry Ghettos. It breaks my heart for those of you reading this who are heart sick about putting your loved one in a facility and don’t have another option. We should be able to trust these places, but sadly, that is not the case. If you have to put your loved one here, advocate for them with all your heart. Don’t let them make you feel guilty for doing it. Visit them often. Not many have the luxury of daily visits like I did, but you must be vigilant and vocal. Remember, the one side looks great, while the Alzheimers unit holds a lot of secrets. Please be careful. God bless!

Presly Snipes: If I could give less than one star, I would. Instead, I will say that the one star is for the male CNA that helped take care of my grandmother when she was there for a month. Unfortunately, his kind and gentle nature was not enough to off set the rest of the issues.The biggest complaint that my grandmother had while she was there, was that the food was terrible. It was often served cold, as was the coffee. On one particular day, there were apparently so many complaints about the food at lunch, that when dinner was served, the meal was extra hot. That was the only meal my grandmother enjoyed eating there.My complaints against Coventry Meadows are much more in number. I start with them not communicating with our family about my grandmother's appointment changes. We ended up calling them to ensure we were aware of when the appoints were to take place. This was after it was committed to us that we would be proactively informed.There was one appointment that my grandmother was taken to, and my mother met her there, where my grandmother did not have socks on. This was during winter. On top of it all, my grandmother's feet were bleeding. I fail to understand how the person helping her to dress (91 year old woman with congestive heart failure) did not see that she wasn't wearing socks, or that her feet were bleeding through her shoes.And then there was the day that I was with my grandmother and there was a bloody spot on her leg. When I asked her what happened, she said her leg was bleeding, and that the nurse/caregiver was aware. What I don't understand, is why it wasn't cleaned up and bandaged.My grandmother passed away on April 14, 2022. Since then, Coventry Meadows has sent 5 notices that she owes them $42.00 for hair styling services. Since the bills were sent to my grandmother's old address and then forwarded on to us from there, it stands to reason that there was a delay in receiving the bill. Upon opening one of the statements, I immediately set the bill up with my financial institution to be paid. The payment was sent to Coventry Meadows within 2 days. A few weeks later, Coventry Meadows called stating that the account was past due and they would refer it to collections attorneys. I advised them that my financial institution had sent the payment, and I went as far as to provide them with the bank's check number and the confirmation number. I also advised that my grandmother had passed away. There were no condolences offered, instead, the lady from Coventry Meadows told me there was no way for her to check through the mail they receive to see if the payment was in a stack of mail somewhere, but that they wanted me to call my bank and re-issue the check. They have subsequently sent more letters threatening action by lawyers. I say, bring it on. My grandmother is no longer alive, and there is no estate to sue. In the long run, it would cost them more than it would cost my grandmother.Again, with the exception of the one CNA, this is by far the worst interaction I have ever had with a company that we entrust to take care of those that cannot care for themselves. I understand that this may be a thankless job, but in our economy, if you are not going to embody the characteristics required of someone to do the job, go find something else.I now completely understand why so many healthcare workers in my area refer to Coventry Meadows as "Coventry Ghettos".
Carly Willert: When I first walked in the door, I was greeted by friendly staff members eager to help! Upon touring, I noticed that it wasn't just the front office staff members that were friendly, but rather everyone was kind, compassionate, and caring! The residents there look so happy and engaged in activities. This is the place I will want my loved ones to reside in if I am in need in the future. Thank you Coventry Meadows for making me feel welcome and more comfortable throughout this process!
Jeremy Egri: The first thing you notice about this facility is it's cleanliness. As you walk through the hall you don't feel like you are in a nursing home with all the friendly smiles and hard working staff. It is always good to see the residents and staff interact with animals. There are always a few dogs that come to visit to brighten their day.
