Syd Waggoner: This was a terrible place from food that was swimming in grease to staff who were belligerent when asked for help.Do not go there. I couldn’t believe how the nursing staff treated me when I asked for help going to the bathroom. The aid told me NO I would have to use the bedpan.This aid when I asked for help in helping me sit-up. She told me it wasn’t her job to do it.I told her she needed to get another job.DONT Go be There.

DONT work here, don’t send you’re loved ones here, stay clear of this AWFUL AWFUL joke of a “health & rehab” center, they literally have bugs in one of their hallways (yes it is vacant right now but they probably will never properly get rid of these bugs they have) they also don’t have a working air conditioner!!!! Only the ones in the patients rooms, had to buy a NECKFAN to stay cool as most days that I was working it would reach almost 80 degrees inside (yet it was set at 69???)Here are the photos of the bugs:

Kelsey Thomas: Grandmother was sent here for rehab while she is fighting cancer on top of having dementia. Place looks nice but it is a joke. No one checks on patients as often as they say they do. My grandma went a week with no bathing and sitting in dry dowels and pee. She would call family and beg for them to bring her water because they wouldn’t. Please don’t send your loved ones to this place unless you care less for them.
Chase Byrer: Always an issue with scheduling, managers are lazy and care little about fixing the issues in the facility even when presented with the facts. They serve food that is not edible to the residents, if they cannot eat it they do not care.
James Elliott: This rehab facility is a total joke. The CNA's are supposed to check the inconsient patients for a bow movement every 2 hours. I am with my wife 10-12 hours everyday. The CNA's are non existent. My wife has sat in fecal material for hours on end with no 2 hour check. She has had a continuous UTI every since she got there. Fecal material not completely removed from her vaginal area. The physical therapy is another joke. They assign a person that is not capable of helping my wife do to their size. My wife weighs 280lbs and a PT that is short and weighs 100 lbs is not going to be able to help my wife stand let alone walk. The kitchen prepares meals that are unsuitable to eat. Avoid Ashton Creek and seek a better rehab facility.
Jill Richman: I have been reading all the comments and reviews and i wanted to give my review. Ashton Creek is a great place to bring a love one and to work. The staff are dedicated to there jobs. The residents here are smiling and very social. There is alot of activities to keep residents busy. Therapy does a great job with the rehabilitation and there are so many success stories. Ashton Creek is a great place to bring your loved one.
Robin Colestock: 35 residents to one nurse.. high acuity of patients that are post surgery.. g tubes..psych patients, 8 plus diabetics.. the work load is overwhelming. Residents waiting hours for their needs to be met. Indiana has no nurse to patient ratio and this facility pushes the caregivers to the top. Resident's needs are not being met.

Rochelle Tollinchi-torres: It takes 15-20 minutes on any given day for anyone to answer the call button being on. The room always needs attention! Floor is dirty, trash from employees supplies on floor, trash not always emptied.
Janet George: My father-in-law had three falls in three weeks at this place. Two landed him in the hospital with head injuries. Items repeatedly missing from his room. Upon moving him out some of his personal belongings had been stolen.
CanterburyNursingCenter facility: Our sister has been at Canterbury about six months. We are extremely pleased with her treatment and with our interactions with the staff. Any questions are answered promptly. The staff is courteous and makes us feel welcome. Most importantly, our sister is happy here. Our family strongly believes that the staff at Canterbury has saved our sister's life, not once, but twice because of their prompt action and we are so grateful.,

morgan jones: if I could give this place no stars, I would. most residents have problems like not getting their medicine on time, not being woken up for meals, frequently falling and much more. many of the staff seems to not care about the people there, they even go to the point of talking trash about the residents in the hallways right in front of them. the nurses and CNAs will also purposely ignore call lights.

Jacquie Downey: The clinical team and other staff are great to work alongside. I see the care and compassion they have for all the residents. I love that we are able to have both a male and female Memory Care wing in addition to offering Long Term care, Rehab, respite and hospice.

Dennis Bushur: My mother had a massive stroke, was immobile, non-verbal and could only communicate by facial expressions. We placed her at Canterbury since it was near family (my brother and I both live 2 hours away). Her room was always dirty, had lots of bugs and had residents with dementia wandering in and out of her room since they did not have a locked down unit. The final straw was when transportation dropped her off at her doctor's office at Parkview North without checking her in at registration. My brother was running late coming from Michigan and when he got to the facility, they said no one had checked her in or seen her. He knew she was there because he called the facility and they said she was "dropped off." He and security searched the building and found her in her wheelchair in the corner of the waiting room at the hospital entrance, not the professional office building where her appointment was, with no identifying papers or a sitter present. The parking attendants had found her and were trying to communicate with her while they assumed a family member was getting a car to pick her up (she had been there at least an hour). When we contacted Canterbury, they did not have a policy of a safe sitter staying with their client while waiting for a doctor's appointment if family could not be there. They told us the driver would occasionally stay there but if they had other drop offs, they were not able to do so. I was very discouraged and disgusted because my mother had no voice and they did not keep her safe enough to prevent this from happening. I highly recommend another facility since the quality of care and caring demeanor was sorely lacking.

Vanilla Sunrise: I have been an aide for 10 years and this is the worst nursing home I have worked in. The staff does the least amount possible. I have come in on my shift and had residents that were soaked and full of poop and some were dried on then like they hadn't been changed all day. The staff is always short and we have to do double halls and less care for each resident. It was so stressful that we went through 3 schedulers in the last 2 months the newest however tells us that she doesn't care about her job because she gets paid regardless is we are staffed or not. The DON has been left several notes and has done nothing to fix any concerns. Staff calls off all the time and do no call no shows and still have jobs. It is overall the worst place I have worked and am glad to be going somewhere else it is so stressful my blood pressure was high for weeks I could not handle the stress any more. So if you ask me I would not work here and I would not put my worst enemy here as a resident.
Selina Shaw: My great grand mother is currently there and the staff provide her with excellent care. The other residents are friendly to her and most of them call her Grandma. She loves it there and IM very glad we made the right choice.

Neil Schafer: Unfortunately what previous reviews state seems true on some matters. Unfortunately it does smell strongly of urine. And as well if u have a lived one there u really need to monitor their food as it's brought in to see the quality. Tonight a folded slim piece of ham on a blot of cheese on a small bun bisquite so hard the bread was I could not bite it. As well the onion rings were rock hard. I witnessed one elderly person out of frustration put it down and bow her head. It was all she had to eat and could not eat it. I understand the short staffness but we aren't short yet on human compassion I hope.
C.V. Bryant: Unprofessional on many levels. Staff or an administrator has their young kids running around at night. Special dietary needs sent from hospitals/ doctors are not met. It's hours before they check on patients, call lights are on for hours. Patients that have incontinence and or bowl issues are left in the soiled garments for hours. Some staff are hostile towards the patients family members if they question the care that's being provided. They also don't always send all of a patients personal belongings and then play games or are extremely rude and beligerent when a family member calls and inquires. Absolutely Ridiculous facility.

Myranda Faulkner: Avenues saved my life not once but twice. The first time I walked in there still in denial and didn't accept being an addict but with the help of the amazing staff and fellow addicts I realized I was not alone in this world and that I did have a problem that needed to be fixed. Unfortunately I'm someone else who learns best by personal experience so I didn't think that relapse would ever happen to me but when it did I knew right where to go to get the help I needed. I'd recommend Avenues recovery to anyone who have the desire and willingness to get clean and to stay clean. Thank you Avenues

Levi Jordan: Best treatment program I've ever been in!! The staff are awesome and they really care for every individual that comes through the door. Staff helped my through a psychosis and my therapist was great helping me through some haunting memories and trauma. Not only did they help treat my drug addiction through MAT treatment, but the one on one therapy was great and the daily classes taught me a lot about mental health as well. Jacqueline Hiatt: I spent 28 days there from January 11, 2023 to February 8, 2023. It literally changed my life. I met so many amazing people and the lessons learned helped me so much. The group leaders have been through so much and can relate to what we are going through. So many of them are former addicts and know what it’s like. The staff there were so supportive and helpful. The only negative thing I can say is that we didn’t get enough food and was hungry some of the time. Besides that everything was good and I’m so thankful for this experience. I’m almost 90 days sober!

But when I got there to check in the RN told me her story and I knew I was in the right place. Avenues is a great place they give you the tools for recovery, the staff is very understanding. Jenniffer Price: I am blessed to work as an NP at a facility with a management team that truly cares about their residents. They serve a specialized population and do it with care and compassion on a daily basis.
David Johnson: Dogs deserve better treatment. My grandmother was sent here for rehabilitation and the staff couldn't lift a finger to help her. The room she shared was at an insane temperature. They made her sit for hours on a portable ventilator. We asked for a regular ventilator and none of them worked (I thought this was a medical facility, so why is all this equipment badly maintained?). This place is a crime against humanity
tasha finley: I can't understand how this place has almost 4 stars, when I stayed wet for days, majority of the staff don't care anything about their patients. People stole from me , barley bathe me, didn't even want me to go to an hospital; although, I had serve chest pains and everyone was getting sick in this nursing home. I called 911 my self! that's when the staff decided to take me to Lutheran hospital. I ended up in ICU. I had pneumonia! I watch a cancer patient of theirs fall right in front of the staffs. The only reason why I gave it one star because they replace my partials and the activity; other than this, It's been about 4 years since I been there and I have not been the same. They were very interested in money. I watch when the state was coming out they start doing their job then and only then. I will not send my dog to live here.

When family or freinds visited the staff was pleasant and respectful. They provide stuff to do activities with residents here as well. Staff is always happy to help you without any complaining what so ever. I've had an excellant experiance and recommend anyone that the stay here will be pleasant. Susan Longbottom: Unskilled facility. Very dirty. Staff does not follow through with tests needed on patient basically they tell you what they think you want to hear. My dad was diagnosed with cellulitis at Parkview hospital and sent for rehab and this facility didnt do anything to help him and said he was fine. Didnt have cellulitis when his legs and feet where so swelled and had sores that bleed. They ignore requests for treatment. Was supposed to get a psych evaluation and did not follow through. Nurses are very short tempered and not helpful.

They are extremely rude at the front desk when I went to drop items off for Leona Resendez. They told me put in on thr floor in a very rude tone. It was the HHCA lady behind the desk. This is the 2nd time they were very very rude. I’m a caretaker and several residents have called me in tears regarding the treatment. I would not send anybody there!!! I may add they didn’t say can I help you? Nothing. They seemed bothered by my presence. If they treat me like that how are they treating the residents? They had no idea if I was there to ask about the facility or what. In fact I did have someone that’s needing rehab but after the 2nd time of being treated with such disrespect I was done!!! I’d like a response as to why I was treated like that. Also I was dressed in my scrubs with my badge on and got no respect. Again they had no idea why I was there. I was also trying to inquire about availability. I didn’t get that far due to the unprofessional behavior!!!

Kody Geyer: Staff is kind and caring. My mother receives the medical help she needs. That being said, she's had numerous items stolen from her. Phone charger has been replaced numerous times. Salts shakers and clothes disappear also. Constant television issues are the norm. Right now the channel is out that the Superbowl is on. This will be the first Superbowl she's missed in decades. It's extremely disappointing there's no effort to stream it for her. It's going to put a sad note on my Superbowl experience knowing she can't watch.

There was trash on the floor, bedside table had a drawer that was full of crumbs/dirt, blinds were broken, holes in closet door, dresser drawer still had clothes in it, sink was coming of the wall, stains on the bed blanket and so much more. In this time they have “lost” her tablet, walker, phone charger, copious amounts of clothing and now HER PHONE effectively cutting off ALL direct contact with me. On top of this, I have to call for days at a time, multiple times a day to get an answer on their phone. Her dr has been trying to contact them for three weeks!!!Do not allow your loved ones to go here. This place is an absolute joke. As soon as insurance cooperates, she’s out of there!!!

I visited several times, the staff was very friendly to us and are taking great care of him. He jokes and gets along with them. Kellie Stine: Arrived here quite traumatized after a stay at Lutheran hospital, when an impaired driver crashed into my apartment, and truly feel like I am surrounded by Earth Angels. The exceptional care is outstanding, and it's been an ideal place to feel whole body and mind healing. Each individual has made me feel as though I'm their only patient and I am beyond grateful. I would recommend Sage Bluff to anyone in need of some extra healing and compassionate care. Being displaced from home is not something I want anyone to experience, but if you can't be surrounded by the comforts of home, this is a wise and supportive second choice. Peace and Blessings and some of the kindest human beings are in abundance on these healing grounds. ☮️

I was coming down the hall to visit my uncle and witnessed her yelling and screaming at the staff. Theres no house keeping staff at all! My uncles room is a complete mess every day. I have to bring a broom and dust pan to clean my uncles room every day. I was there over the weekend and the floor staff was made to go cook in the kitchen. I am really honestly debating moving my uncle out of this facility. Ever since this new administrator took over the place has gone down hill.
Nancy Schantz: My dad was at Sage Bluff for about a year. In that time I saw a very big improvement in the staff and how things were run. They got a new director of nursing, and she is really on top of things and cares so much about everyone who works there doing a great job. She was always very responsive to me when I had questions or concerns. When my dad was declining rapidly at the end, the staff members were absolutely wonderful to my dad and our family. There was a special hospice room where we could come and go to visit my dad. People were so kind, and they did everything they could to keep my dad comfortable and to give the family assistance. Their care could not have been more compassionate, professional, and skillful. The persons who work at the front desk were also very helpful. We are very thankful for the good people who made my dad's last days comfortable.December 26, 2020--When I wrote the original review, I neglected to mention that there was one CNA who really went above and beyond for my dad. Her name is Megan, and she deserves a shout-out for all of the good care she gave. Thank you so much!
Larry Mitchell: Inconsiderate management and mishandling of a dear friend. I learned recently that a great friend of mine was transferred from a another sister facility to Sage Bluff to be closer to family which would allow for more visits. My friend has dementia like symptoms and requires more attention than normal. Unfortunately his trustworthiness of others has allowed him to be taken advantage of and put him in a financial hardship, which he may or may not completely comprehend. With the help of loved ones he was able to obtain some government subsidy to finance his stay. Because of his condition he has tendencies to do things atypical such as obtaining newspapers or other items that may not be his. These are relatively harmless events, but are not reflective of his past “self”.This is where the sadness and disappointment arrives.Recently my friend was evicted, with administration citing a couple of reasons, one for obtaining a newspaper from the entry way (which he is not allowed outside the facility without accompaniment, but he didn’t leave the entrance way) and second for taking salt and pepper shakers from the dining area. Again these are not normal of his character but a result of his condition. His previous residence understood his condition and made accommodations to care for him. Sage did not make any accommodations and simply downplayed his condition and swiftly evicted him rather than trying to learn and treat his condition. My conclusions wether right or wrong is they simply did not receive enough benefit from his government subsidy and needed to make room for a more favorable payor. Something the health board will be made aware of. There was absolutely no compassion for his situation and when family tried to contact the administration they were stone walled by staff and the nurse administrator refused to schedule time or discuss the matter with them. I would be very cautious before you send any of your loved ones to Sage Bluff.

It's so many bad things that happened to her in there, I don't have enough space to type all the things that happened.. The good people that genuinely cared were def far and in between..

First time going to see my mom(she got moved on a Friday I went the next day) my mom spilled a drink on her bed I asked for her bed sheets to be changed . The nurse brought the sheets to the doorway and hollered for me to come get them. Really? I dont work there and who just yells for a patients family to change the bedding? I got my mother up into a chair and changed her sheets and needed help getting her pulled back up turned the call light on no one came my kids had to find someone. The staff was out in the hallway talking about the residence. The nurse was passing meds and talking to her kid on the phone, a big mistake could of been right there. Another cna was walking the halls in a spaghetti strap tank top her bra and back and all was showing. Another cna was wearing slide on sandals. This place is a damn joke for real
Robin Colestock: I worked as a nurse on their rehab unit. I loved the job at first.. then quickly realized how bad it was. I understand healthcare facilities are a business.. but in my 20 yrs of nursing experience in fort Wayne i have never encountered such blatant disregard for patient care. It was all about the budget and cencus. Patients were not getting the proper care, dressing changes not done per shift as ordered, and management didn't care. They ran a skelton crew of CNA's, so patient care lacked. When a nurse is told to "not send anyone to the hospital because our census is low and we can't make payroll" Well that was a deal breaker for me. The administrator and admitting "person" (who is usually an RN) and in this case not even a nurse to tell a charge nurse basically to ignore medical needs made me re think even being a nurse in today's " it's all about the money" healthcare system. I resigned d/t health reasons, but i was ready to quit anyway. We get it.. your a business. But when your business is taking care of people the bar is raised. Sage Bluff fails to even attempt to reach the bar. If you have a loved one in any healthcare system... go every day and ask a ton of questions. Be an advocate for your loved one. Be a constant presence. Great nurses are picked at and ridiculed, while lazy " I don't look for work" nurses get their asses kissed by management. Wish i never went to work for them.
Diana Swartz: Sage Bluff has a beautiful new building and nice front office staff that seem willing to help, but trying to get an update from a nurse over the phone is a different story. They are very unresponsive and have hung up on me several times when I had to ask them to please help my father and give me an update on how he was doing. This should not be the case. I do have to say that every once in awhile I get a good nurse taking care of him that spends lots of time answering my questions, but this is not the norm. My mother also gets no help for her dementia. This is NOT a memory care facility.
Autumn Smith: My grandmother has dementia is diabetic and recently was put in sage bluff for rehabilitation. After having stroke like symptoms and very bad sores with infection on both legs. Sage bluff may look nice but its staff does not match that appearance. When arriving to visit myy grandmothers bandages from her sores were sopping wet with puss and the bandage had unravelled. A housekeeper was changeing the bed linin and said my grandma could not get back into bed until her bandages had been changed and that some one would be in to do it. I stayed for about an hour. Nobody had even came in. So i proceeded to the nurses desk to ask when her bandages would be changed. Mindd you their were 7 people 3 nurses 4 aides just standing around. I asked the first nurse who said thats not my patient and pointed me to a women named Lisa. I asked lisa when my grandmas bandages would be changed. I got immediate attitude. She stated her bandages dont get changed until night shift and that my grandma unravels them and thats why they look like that. First off i never said 1 thing about them being unraveled. 2nd my grandma has dementia shes confused. 3rd if her care plan states that her bandages be changed 1x a day then apparently someone should be talking to the doctor about doing it more often. They are seeping puss and my grandma was told she couldnt lay down until they got changed. This nurse lisa continued to have an attitude and proceeded to talk about me as i walked away. I will be contacting the director of nursing. Places claim to be so understaffed but my grandma is sitting alone in her room with puss leaking out of her wpunds and bandages and then we have 7 workers sitting around a desk doing nothing. Bullshit bullshit bullshit. Sage bluff i am watching and i will be contacting the propet people . do your jobs

April Elliott: Deeply saddened that the premier rehab facility in Fort Wayne was lacking in all aspects of care. This facility came highly recommended so Admitted my Mother after a fall with fracture for therapy service. I was unable to tour the facility, but with the recommendations I assumed she would receive 5 star care. Well we all know what assuming does. I walk into the beautiful Private room, and my mother is laying in her bed writhing in pain stating they’re not giving her pain medication because she admitted on a Saturday evening and they have not come from the pharmacy. After speaking with several staff members was finally able to get the pain medication she desperately needed. I had to assist her to the restroom as no one was answering her call light, and this was all within less than 24 hours of her admission. I could go on and on. Needless to say, she is promptly being discharged from this facility and taken to a facility that I know, will provide her the care that she needs! Shame on you Kingston, for allowing this type of care in your five star facility!!!!
Tonya Perry: It seemed like a nice place at first and the therapy people were really nice. But in the two weeks that my mom was there things got worse. Her clothes were stolen, her room was in disarray and they expected her to go in her pull up insted of taking her to the bath room. When asking someone to make my mother's bed she just stared at me and never responded.
B Brown: I've been pleased w/ the care Kingston has given my mother during her rehab stay. The nurses have been very willing to discuss her care with me, respiratory therapy has been great, and the social worker, and PT/OT/speech people have been very kind & honest with their evaluations. One small example: at another rehab place she was at earlier, they kept forgetting to put her hearing aids on the charger; Kingston has been almost perfect at remembering that nightly. For my mom & her poor hearing, that makes a big difference.

Jason Hartman: The care for my father has been shameful. This place charges $289/day and that's just for room and board (no medication or therapy included in that price). At almost $9000.00 per month, the food is terrible, low in nutrients, almost entirely all beige, and cold when it arrives (EVERY TIME). A typical dinner is a cold hot dog. It takes forever to get help when he rings his buzzer. Last night he had to threaten that he would call an ambulance and finally the staff helped him. When I visit, I have to chase down staff and convince the nurses to help. When any of us visit, we feel like we work there because, well, we do. We were also informed on a Friday afternoon at 3pm that b/c my dad's physical therapy isn't progressing enough, his Medicare would be stopped and his last day at the facility would be the following Wednesday. The communication is horrible and really non-existent. We were promised early in the process that we would be given plenty of time should this ever occur and now we're being forced to scramble to find a new facility with very little notice. Save your loved ones and yourself and don't place them here.
Diane Reust: I was recently at Kingston Care Center for rehabilitation. I highly recommend them to anyone who might need rehab. The rooms are nice and clean and the staff was all very nice and took great care of me. The therapy departments were wonderful and patient with me. I wish I could give more than a 5 star rating. If I ever need rehab again I'd definitely try to go there again.

Then they try to justify poor care rather than fix the problem. I just talked to Shawn, one of the head nurses, and I am so frustrated! If you want proper care for your loved one don't send them to Kingston. Pastor Paino would be so upset.
Doc Farmer: Had a family member at Kingston for a little over three weeks, after a fall at home (nothing broken, thankfully!) and a (thankfully) short stint at Lutheran Hospital, a facility that was really lousy. But that's the subject of a different review. Anyway, Kingston was OUTSTANDING. The first few days there were under a flu restriction, meaning folks couldn't leave their rooms for meals in the dining room, which was understandable, so all the meals were delivered. A few of them weren't as hot as they could've been, but the staff did the best they could under the circumstances. Once the dining room opened up, the food quality and temperature rose precipitously. The general offering at each meal was pretty good, but they also had an a la carte menu if she wasn't in the mood for that. Meals were hot, delicious and fulfilling.But what REALLY set Kingston apart was the high level of quality care, and the caring attitude of the nursing staff, the physiotherapists, occupational therapists, speech therapists and general support staff. All of them were top notch. All of them were very respectful of the patients, and treated them with dignity and a respect for their independence. Never talking down, but always talking clearly (and sometimes loudly enough for the harder of hearing) and making sure everything was understood. They don't "seem" to care, they actually DO care. And it shows.There are plenty of choices for rehab care in Fort Wayne. You could do a lot worse by not choosing Kingston, but I honestly don't know how you could do better by choosing another facility. Kingston is an EXCELLENT choice for rehab care, and I recommend it highly.
