Brookdale South Bend: what do users think?
Nancy Kieft: 5 years ago we were so happy with the facility. As time went on and staff changed I was very unhappy. I'm not going to go into detail, because I respect a few employees at the facility. I would look for other options.
Stephen Bubanovich: Attentive staff, nurses and medical care. It is very much like a smaller home environment. Billing is accurate and they handle any insurance paper work well without errors. Food service accomodates the resident’s needs. I would give it 5 stars if they did some resident bathroom remodeling and added more staffing to help improve laundry services.
jennifer grant: Care, compassion and excellence. Best caregivers!! David is top Chef! Always making residents recipes and accomodating their wants. Just ask about Polish Tuesday's and the newest request he made was baba gunnoush!
Jessica Whitehouse: A lively assisted living with a compassionate staff and fun environment.

Wood Ridge Assisted Living: what do users think?
David Shirk: This is scathing but accurate review. At the top, administrator is narcissistic, petty, racist snowflake. President is the same. Food is the lowest quality not prepared properly. Staff is rude and incompetent, except for one. An Angel. Good luck with getting your own meds. Equipment is broken down, grounds unkept with countless excuses. Look elsewhere unless you want to hear whining about state and the attorney general coming. And can't forget the bedbugs.
Ashley Weidman: One of the girls who works there was beyond rude. I walked right up to the front desk because I was delivering a grocery order. I asked her how to get to the second level. Her response, “you go to the elevator & press 2…” I asked her where the elevator was because I had never been there before and she said “well why don’t you look behind you”. I have no idea why she was so rude, but it was awful and so uncalled for.
Lorraine Rattazzi: I was very excited to be going to Woodridge and was led to believe it was just what I was looking for💁🏼I must say, there were good and bad but overall I was very unfulfilled especially when I actually only needed basic help and follow through regarding getting physical therapy and referrals to dr that could help with pain and surgery needed on my hipI was there a little over a month and felt more anxiety and depression than I had in years bc Nothing was completed with regards to what was expected or promised!Ii found them to be very unorganized and although I liked the dr I met2 time there. She was extremely inconsistent with showing up and the follow through on everything was very dismalMost residents there just seem content with ? Whatever?🤷🏼‍♀️And exist bc no other options or helpThe facility could be sooo much better but the overall consistency was the only thing you could count onThe menu is pathetic but not kitchen staffs fault bc it’s obviously the cheapest, most generic grade of food that is purchased!!Fattening empty carbs and rare fruit or vegetables worth eatingThe ownership 🤷🏼‍♀️Food…-1Cleanliness,…8Activities ,… 1Doctors,…2Psychologist,… 11😊CNAs,… 5Maintenance,…5Coffee,…0Rooms,…2. Tyron Hunt: The people and staff re helpful
Danielle Brown: Have had to report this place to the state multiple times. They constantly mess up medications. Also, they kick out residents without notice
Patricia Erlacher: I've lived at Wood Ridge for 2 years and have found the staff to be genuinely very willing to be of help for any needs we may have. Always willing to listen. In my opinion the food is extremely good! Come tour and see for yourself!
Walda Simmons: Wood Ridge took me in when I had nowhere else to go. The staff always helps me with things, like finance and insurance things I can't handle on my own. There is always a quick turnaround when I put in a maintenance request in the book which is nice. I like to sit out on the back deck, it is so pretty. I'm sad when good staff members leave sometimes because I miss them. I like that they are pet friendly because I can have my cat with me.
Matthew Glencoe: Great team who really cares and some really sweet residents make it a great community.
JD H: Charley (resident) has sharp dance moves and shares a fist bump with children visiting, whipping a dollar out of his clip and smiling infectiously. The staff is top-notch.
THE 1: Crazy ass old ppl who drink daily, get hammerd there often
Joe Agostino: The entire staff is personable and caring. They treat your family as if it is their family.

therese humphrey ball: Great place good standards
Carol Woods: Not very well taken care of outside. Rooms could use updating, carpet is scary looking, bathrooms need work, short toliets are hard for the elderly. Staff is very friendly and helpful. Food is good, breakfast could use some improvements
Hulbert Colwell: Very clean.
Patty Cummins: Nice assisted living community, name changed slightly. Handicap parking and handicap assessable.
Steve Link: My mother lives there and she downright loves the place ,nuff said....

James Gronk: I've seen them all and this is one of the nicest, the staff is fantastic checking on Mom three times daily and the maintenance is spotless. I highly recommend this place for all your caring needs.
Carol Hillman: My mother spent time in the rehab/nursing home side and later in their assisted living. I cannot say enough good things about their facility and employees. They go above and beyond.
Bonnie Daker: Very beautiful facility and my friend who lives there is quite happy and content. I wish my mother would have had such a wonderful place to live her final days!
Karen Vincent: This place should get in trouble with Medicare as they pick and chose who they accept for rehab. My mom was turned away because they said they don’t think she would be a good fit for their facility. My mom lives in a beautiful house in a community close by their facility so being close to her home was important to her. I just think it’s a shame and this type of pick and choosing doesn’t happen where I live in Florida. If a doctor writes a order for rehab with physical therapy and a bed is available in a facility it should be their right to go where they want as a patient and a customer. I would never recommend this facility.

T L: The director of sales and marketing Marla Schroeder is a NASTY, NASTY Facebook STALKER...
Bradley Dickey: I've been living here close to 4 months and I absolutely love it. Extremely clean facility, amazing employees and it really feels like a home. I highly recommend it for someone that needs that little extra help in their daily life.
Alison Peters: The staff is of good respects, but there is new charge employees that show favoritism and carry improper rules and connections within that of the community both employees and patients. My interest in this place for my aunt has been lost due to contact and identification of they of the community.
James Barcus: If I could give this place a negative I would my mother in law was in the hospital over the weekend and they failed to notify anyone there response it won't happen again and we need another months rent no nurse on nights be careful with these folks
Ashleigh George-Henderson: I love this place tight little family
Wendy Dufford: Don’t bother researching Grand Emerald. They up charge every month stating your family member needs more care and then don’t provide it when being paid for it. My family members feet were so swollen she had to be in a wheelchair. Didn’t know that they hadn’t visited the doctor about her feet nor did they help my grandmother with basic grooming for months! Her toe nails were so overgrown that they wrapped around her toes. I’m not even embellishing the horrendous condition my grandma was in. Then they billed me for days that my grandma wasn’t even living there. When I asked about it, they didn’t respond and turned me into a collection agency. Can you believe that? Now they poked the bear. I tried to copy a photo in here but it’s not allowing me to. If you’d like to see them, feel free to email me or look st my review on yelp. Do yourself a favor and stay away from grand emerald.
Tim Yazel: Very nice retirement community. Great dining room. Nice rooms. Many activities on and off site.
Rob S: A loving and caring staff !
Cheryl Wright: Great place
Donna Grove: Mother-in-law has been there 2 years. Very nice place, but family members seem insignificant to the higher staff.
Andrew Cuthbert: Nice place but staff could be better.
Gina Rath: Great group of people- including Regional management-anyone would be happy with this community being your new home!
robert bjorkland: Warm and caring
Leann Sanders: Grand Emerald Place is a fabulous place to work and live! We live our companies mission, vision and values everyday and promote Compassion, Humility, Integrity Excellence as well as FUN. I enjoy working here and believe in our quality care so much I have moved a family member of mine in!

KeaNichole Bennett: carestaff named who said i had a gun lied on me i recorded you and jennifer you was wrong not letting me see my family them 2 care staff who brought my grandma and aunt down was in the for asking any nothing what i do for i living was wrong for even asking me anything about me i know of you abuse my family my grandma told days before knowing none of you ladys was scared to leave from my family i also ya was afraid she was going to me tell me i recorded everything before i love my family
kelly gates: My parents both live here. The staff is kind, professional and helpful. I appreciate that during the pandemic they have been diligent in containing outbreaks and honest about the unfortunate happenings. The rooms are spacious, clean and modern. Mom doesn't complain too much about the food. Dad loves it. Although they are no longer married the staff has made our unique situation work! Dementia/memory care are difficult path to travel but I feel that Morningview does a great job. Thank you!
Ken Lindow: We have our grandma in Morningview and she has been very happy. The staff is friendly and warm, from the front desk, throughout the building. The activities schedule is full and the resident artwork decorates the walls. They paint, craft, and create, listen to music, play games, play trivia and have ice cream socials. I know all of this because grandma has been a part of the Morningview ALF family for nearly 3 years. The facility has a clean, industrial-chic look. Many rooms are newly renovated. We are glad Grandma has had a pleasant environment in which to spend her golden years.

Wellbrooke of South Bend: what do users think?
Jazmine Hunter: Great staff and service.
Brandie Mejer: If they decide to transfer your family, it is made weeks in advance and family is barely given a 48 hour notice. Family member was sent to a terrible facility almost an hour away from where all of us live. For profit health care is disgusting, but that's the American way I suppose
Jill Cress: Very disappointing experience with my grandma. It was very hard to reach anyone by phone and they were very bad about returning messages. Had previous experiences that were great but not anymore. If you are needing rehab highly recommend looking elsewhere.
Tara Mcgraw: My mother in law stayed here after leaving Memorial great staff and experience
Lisa Buck: My mother had back surgery and she's at Wellbrook of South Bend and this place is awesome. I am so pleased with everything, from her care to the food to everyone that is helping her on her road to recovery. Everyone is so nice and helpful. She loves her room and all the people. I don't think I would want my mother anywhere else. The grounds are beautiful. She loves seeing the ducks come up to her window. I just want to thank everyone for your kindness and care you are giving my mom
Victoria: Pleased with the care my mother has received. Excellent nursing staff and management.
Terry Scheibelhut: WONDERFUL PLACE !!!! THE STAFF IS EXCELLENT, THE FOOD IS GREAT. BEAUTIFUL CLEAN AND LOOKS LIKE A HOTEL.GREAT ACTIVITIES AND THEY TREAT YOU LIKE FAMILY. THEY ALSO CALL THE FAMILY MEMBERS EVERY WEEK TO TOUCH BASE. THE BEST NURSING HOME IN SOUTH BEND. I GIVE YOU 10 STARS !!!!!! GOD BLESS YOU ALL WELLEBROOK. YOU ARE THE BEST !!!! TERRY ANN, SISTER OF GARY SMITH
Carol Hillman: Apparently, the only way a Wellbrooke can get a good review is from their employee Said! This is the 2nd one I looked at and most of the "fives" are from employees. Not a place to take your family members.
Annette Stewart: The staff is great with their residents, they are caring and very friendly. They make the residents feel special. The food is good as well.
Brooke Hummer: A family member was there about a year ago. Due to the lack of wound care among other things she got an infection. After bringing this to their attention by my sister who is a nurse they did nothing about it. My sister ended up calling for an ambulance and she was taken to the hospital where she stayed for several weeks. Would not recommend if your loved one is recovering from a surgery or has an open wound.
Caley Nixon: Wellbrooke of South Bend is a great place to work. I have been in the health care industry since college and this has been one of the best career moves for me and my family. I get to hear numerous stories from residents, family members, and visitors about how our services, employees, and campus go above and beyond expectations. I look forward to many more years of serving the community and residents at this campus.
Emily Nieboer: I have worked for Wellbrooke since 2 weeks after we opened... this is one of the best places to work because I get to hear lots of stories from the residents.
Derek Ressler: Best place in St.Joseph county for short term rehab., Respite stays, long term nursing care and assisted living.
ANDREA NICHOLS: I was invited many months ago, to go to Wellbrooke, for a luncheon and a speaker. IU took a friend with me, and we had a great time- I met several women that were interested in the same things I was and my friend and I had a great afternoon. As soon as I am recovered from my recent surgery, I hope to revisit Wellbrooke and get some good information from their speakers and to meet again with the people I met so long ago-The staff is great, considerate and caring- I recommend Wellbrooke highly.
Angaleana Smith: Wellbrooke has been a wonderful place to work at. Trilogy goes above and beyond for both residents & employees. I have never experienced a company that feels less like a company and more like a family.

Holy Cross Village: what do users think?
Taine Conboy: I have been at Holy Cross Village for six years. My husband and I moved into Andre 311 in 2017, and he died a year later. I have made wonderful and supportive friends, and look forward to many more years here!!!
Barbara Ziliak: Staff members are very focused on the comfort and care of residents. Because of their efforts, we are able to enjoy peace of mind and many friendships among residents. Andre Place is designed for independent living. Our living here has made our lives so much easier as we age--thanks to a beautiful environment and a high professional staff.
Doug Archer: My experience since moving to Holy Cross Village late last year (2022) has been nearly perfect. The residents and staff have been warmly hospitable, the services (especially meals) excellent, and the activities on campus and in the broader community numerous and varied with no pressure to participate beyond my interests.
Louise Fessenden: Loved it from the first day we moved in -- and still do, after 15 years!
Patrick Campbell: My parents moved to HCV 15 years ago and I've been incredibly fortunate due to all the individual care shown by the entire staff for my Mother and Father. My Dad passed in 2020, and the continued support for my Mom and handling a celebration for my Dad has always exceeded expectations.
Marilouise Hagenberg: Great place to live!
Carol Brach: My experience here at HCV has been much more rich and full than I even imagined when I moved here 8 years ago. Security, yes, beautiful surroundings and facilities yes, free from housekeeping and maintenance, yes. But so much more. This community provides wonderful support to residents and beyond to many local charities. I enjoy the spirit here. Fellow residents and staff members care for each other. It’s an active community rich with opportunities.
Stephen Dunham: My very first visit here. Listened to Bishop Frank Gray talk about the Dead Sea Scrolls. What a great talk. Such an amazing place.
stevie mark hara: Beautiful, comfortable and friendly place to work
K “Zimber” S: Working here was always delightful. Staff and management are professional.
K Le: Home! 💕
1967sugarlump: Very beautiful place
Rob G: Ants in my mother's room that were not taken care of until THREE days after staff was notified. She was treated like a child by two of the aides. When she put her light on for a nurse, the average response time was a half hour. Administration very rude and gave false information to our family. Not impressed at all.
Jan MacLean: My father lived in both the assisted living and “memory care” (Quinn Unit) for almost a year. There was no transition support when he moved to assisted living from another community in Indiana. He wasn’t seen by a nurse for the first two days of his move to HCV. We ended up hiring care givers for six hours per day to ensure that he wasn’t just left in his room alone.... sleeping his days away. His move to “memory care” was a huge disappointment. He literally begged me to “get him out” stating numerous times that “this is not a good place.” He is a retired physician and did have lucid moments - he did know what he was talking about. The unit was not clean and was under staffed. During difficult moments I was called at home, which is exactly what I would want to happen, but staff was not always professional in the manner with which they referred to his distress - once even laughing about his agitated state. My family and I felt it was necessary to be a physical presence frequently as we did not trust that his care would be of a high quality.
Thelma Donahue: My husband and I moved from Coral Springs, FL to Holy Cross Village after having looked at similar facilities in Florida and Georgia. We came to visit HCV at the urging of our daughter and made a decision to move here on same day. Almost ten years later, I can say it has lived up to its promise. In addition to the usual amenities (lovely dining room, exercise therapy, social programs, etc.) there is a spirit here that is unique. A camaraderie among residents and a sense of looking out for each other is evident in our everyday lives. Our dedicated staff is always willing to help with any concerns we may have.
james whitehead: Our initial experience of Holy Cross Village was part of a visit with a friend from our earlier years of teaching at the University of Notre Dame. Being in our early seventies at the time, we had thought of making a move in our eighties, but after meeting other residents and staff members, we moved in sooner! We appreciate the range of events available to us since we are now part of the larger Notre Dame community, including football games! We benefit greatly from lectures being offered by residents here, as well as by university colleagues and others from outside Holy Cross Village
Lenore Cassady: My husband and I moved to Andre Place at Holy Cross Village on Nov. 1, 2007. Now some eleven years later, I realize what a great decision it was. The residents as well as staff are so nice and always have a smile and stop to say "hello!" The dining room is lovely and the food is very good. I personally know the benefit of continuing care, as my husband had to go to the Quinn Memory Care Unit located on the same campus. While it was hard to have him gone, it was so easy to walk from our building the Quinn.
Iz2047: If 0 stars was an option... My father had two fused hips and finally took his first step to recovery with his first hip surgery. For anyone who doesn't know, after hip surgery it is very important to get up and walk.-They just stuck my father in a room and left him there.-Brought him food but gave him zero, none, nada attention.-Was brought there in a surgery gown on Friday and was still in it on Sunday.-Never assisted him in daily routines and grooming-Room was bare and unsuitable for someone of his condition.-Never assisted with walking or physical therapy.-No staff available to help him in his movements.-No respect for patients who have passed.Do yourself and your loved one a favor (if you love them). Stay out of this place

Trailpoint Village: what do users think?
ardell williams: wish i could add a picture. this place is terrible and when i say terrible i mean bottom of the barrel! i have a video of nursing staff and dietary drinking on the job !
aimee valentine: My Husband is there now and it's awful! He can't get his meds or get help with emptying his colonoscopy bag.. They dont even have the right size to fit him. The paint is coming off in his room. The floors in his room are disgusting! He was sent there from the hospital. I should have did a lot more investigating before he went but just looked at the pictures on the website and it was beautiful to bad that it was NOT TRUE AT ALL!!! If you or a family member is recommended to go here DON'T GO try and find somewhere else.
John E: Awful, awful, place. If you have a choice I wouldn't take a dog there.
Chris Hager: My dad was sent here after a below-knee amputation, for rehab. He was summarily sent home right after the mandatory 2-week COVID-19 quarantine, when the insurance money ran out, without having received any real therapy or rehab. He was sent home unable to care for his own basic needs, and before his wound had fully healed (creating a very dangerous situation). He had to be taken, by ambulance, back to a hospital 3 days later and is now in a different rehab facility getting actual therapy and improving his strength by the day. We would not choose "Trailmix Village" again if we had the choice to make over again (which we did a few weeks ago, hence his being in a new place this time).
Liz Beelby: Despite assurances my mother would receive excellent care in the rehab facility, time after time she was neglected and ignored. I would not recommend this facility for your loved one unless you don't mind them sitting in their own bowel movement for over an hour. She would page the nurse but the beeping would go unanswered for hours. Even my father couldn't find a nurse anywhere in the wing. Very poor care.
Kevin Clem: As the Executive Director, I wanted to thank all of you for your responses and your feedback. At Trailpoint Village we take pride in our quality and our ability to take care of the individuals we serve. Being new to the campus, I am impressed daily with the clinical team's ability to navigate the complexities of resident care. I would encourage all of come in and tour our community. Individual who have concerns please know that I take concerns very seriously and would love to talk with you about how to improve our operation and resolve any service breakdowns that we have had. At Trailpoint Village we live our "CARE" values.Compassion, Accountability, Relationships, Excellence. With Respect,Kevin Clem, Executive Director
Sheryl Gerald: I went in today to visit a relative. The conditions were horrible. You can't slap some paint on the walls and say you have fixed a dump up. I would not take my dog there. I guarantee my relative will not be in this hell hole long.
Sam Stout: This place has a very strong clinical team and an awesome new executive director. Also the new renovations look wonderful.
homeinwi: Personal items go missing with no accountability.Lack of staff attention during working hoursCleanliness is low. Trash bin emptied daily but don't expect the rooms to get swept or moped. I cleaned the bathroom myself just so it wouldn't stink so bad.Simple maintenance in the rooms is not done. Screws falling out of the beds, toilets running non-stop, wall switches not screwed in. Simple fixes not maintained.Management met with me and did nothing about concerns of care including falls.
Patty Cummins: Nice elderly folks home.
