Michael Lane: I would not recommend this place.My mother was being discharged from the hospital and we had only a few hours to decide between a handful of rehab facilities. West Bend had the highest Medicare rating of what was presented to us and some positive reviews on google, so we went with them. A choice I regret terribly given my mothers care and quality of life at the facility. I later also figured out that several of the more glowing reviews had been written by employees of the facility… Or its a crazy coincidence. For example the review by Grant Carter praising the staff and the owner replying back happy about their “positive experience”. At the time someone with that name was the maintenance guy and he did a good job and was very responsive, but I was really surprised to see him writing reviews.The biggest problem I saw was that my mother was left in a wheelchair several times when she was in pain from being in the wheelchair. One time I left her, came back a few hours later, and she was nearly crying from pain and told me she had asked to be put back into bed for several hours and no one would get her back into bed. This caused me to lose all trust in the facility and I ended up spending 5-6 hours a day there. If it hadn’t been a short term stay I would have moved her immediately, but I was advised moving can be detrimental to physical therapy, so we decided to bear it for a few weeks. Management did attempt to address my concern, but my trust had been broken and couldn’t be restored with a promise without seeing it in action.Next, some of the CNA’s in the facility just didn’t care. One day my mother needed to use the restroom, and the CNA told her “just go in your brief”. Which to me is not a valid thing to say to a resident. I can only assume if I hadn’t been there to demand care for my mother she would have been left sitting in her own filth for an extended period. When my mother would use the call light it would often be 30 minutes plus until she got attention.The food looked like something from a prison. One day in particular they served fish that was strangely firm and inedible. Occasionally there would be something my mom could stomach, but overall it was the lowest of low quality. I’m sure it all technically meets minimum requirements set by the state… The food never arrived to her room warm. Half way through her stay the dishwasher broke and they were serving everything on flimsy paper or styrofoam plates which they would often microwave. Hopefully it was microwave safe styrofoam.Additionally, she was put into a room with a roommate rather than a private room for her rehab. The roommate continuously watched her television at a very loud volume, and my mother was unable to get good rest which I am sure was detrimental to her physical therapy.On top of everything her discharge was a mess and I received inaccurate guidance from the staff. I was told no facilities could get her PT/OT and Nursing at home, so we settled for someone who could do PT only. We started PT only and I was surprised when a few days later I got a call from a home health company indicating they could provide services. I asked why they had told West Bend they couldn’t, and they indicated they said they could, it would just be a bit delayed to get started. And waiting a week for full PT, OT, speech, and nursing was a hell of a lot better than getting PT only. I was also told very inaccurate information about what her insurance would cover upon her discharge. So my advice is do not go on what the staff here tells you, you have to call your insurance company and figure things out on your own.There were some staff members who obviously did care and did their best. But sadly they couldn’t compensate for the overall bad experience.If you care about your loved ones and have a choice, don’t let them go here. Don’t just go by the Medicare rating, as it really doesn’t reflect the true nature of this facility.
Amanda McClain: The cottage and Liz (Elizabeth) the social worker of the cottage in particular has been a great help to myself and my grandmother. Going above and beyond to help make her feel at home even during difficult times. I was shocked to see the previous review posted. In no way is Liz (Elizabeth) racist in any way shape or form and if you got to know her and her family you would come to realize how ridiculous that statement is. Liz has cared for my grandmother as if she was a member of her own family and we can not thank her enough for that. Liz has never made us feel any less based on the color of our skin.
Tom Stantz: Im very happy with the care west bend is giving my mother. I had chose them because my grandmother stayed there and I was very happy with everything they did with her. And the care they gave her. The staff was and has been amazing for both of them. Thank you for everything you all do. My mother is very happy staying there.
Jennifer Thompson: My mother has been a resident in the dementia care until almost a year and a half at West Bend. The care she has received has been above and beyond my expectations. The love she’s shown is a blessing to me and my family. Liz treats her and the other residents like a member of her own family.
Purple Flow: Elizabeth (Liz) who is over the memory cottage is very unprofessional, insensitive and needs to be dealt with accordingly. How she communicates about our loves is terrible. Our last few encounters, she’s made me feel as though she is racist. I will be reaching out further to see what action can be taken.
W Renae Bridges: The food is terrible, there seems to be no consideration for all the diabetic residents and their need for a low carb low sugar diet.They never have enough staff. The nurses sitting around the nurses station are often consumed with surfing the internet..The nurses ignore the patient room call lights.

Donna Rock: My Mom died at the hospital after I pulled her out of there. She hated it, no respect was given to her there. When the paramedics came 2 patients were on the floor, they fell...no one was around. The paramedics were shocked! This rating is for her.
Monica Solina: I wish I had read these reviews before I ever let my husband be taken here. He was transferred here from Goshen Hospital after battling Covid in the hospital. They are not doing physical therapy with him, they have done it twice in 6 days, I was told at least once a day every day. I have tried to get ahold of someone besides whoever it is the answers the phone and not gotten a call back. I am in the process of getting a hold of insurance so that they know not to pay as he is not getting what they are supposed to be providing. I would not send anyone that you care about to this facility !!!!
Mike Angelo Fades: DO NOT TAKE ANYONE HERE! They said my mom had covid when she didn’t! They treated her bad and she was getting sick! We took her out and she is doing way better at home! They never let us talk to her until we took a personal cell phone to her. Very unprofessional!

Noob Noob: So I want to start with the fact that I do not usually write reviews for places, but cardinal is a special case. My mom recently was admitted to this "nursing" center and "rehabilitation" after going up to the hospital due to her inability to walk after falling.Before going there, the hospital she was at took very good care of her, they cleaned her, fed her, and provided medicine for the pain. Now CARDINAL on the other hand is a completely different story.It hadn't even been one day in the nursing home before she called me up on the phone crying, since they did not help her to the bathroom, and was now sitting in her fecal matter in tears. THIS IS UNACCEPTABLE.Not ONLY is the staff UNCOMPASSIONATE they're also THEIVES, my mom came to the nursing center with 2 lotions specialized in getting rid of bedsores due to sitting for a long time in a hospital bed. They proceeded to use both tubes on other patients! Now she has MORE bedsores than when she arrived due to lack of care, and NO LOTION to put on the sore.After some talking with my mom we decided to give them another chance and allowed her to stay an additional four days. In those four days, a physical therapist only visited ONE day and did exercises with her for FIFTEEN minutes before moving on to the next patient.I understand they have more people to get to, but this is a rehabilitation center to help my mom get better not sit in a bed 24/7 and degrade into a worse state than she was previously.If you have a better option than this place, I would take it. As the staff does not care for anyone in this place. The amount of malpractice thievery that those UNSYMPATHETIC SLOTHFUL nurses have committed in the past week is atrocious.
Gina Beery: My Mom had a massive stroke while on vacation in Florida! The team at Cardinal "moved mountains" to quickly help me make all the arrangements to get her back to Indiana, to get the specialized care she so desperately needed! The therapy team at Cardinal is the BEST I've ever seen!!! PT, OT, & ST went above & beyond working with her! They were kind, caring, & professional! She loved them! I can't say enough about the phenomenal care she received while there! I would highly recommend Cardinal to anyone who asked! :)

The aides are caring and attentive and the nurses are kind and helpful.I'm also an employee at Cardinal. Knowing long term care the way I do, it says a lot that I would trust Cardinal with the care of my family member.No facility is perfect, but at the end of the day Cardinal cares about the residents and provides loving person centered care that rivals other facilities.
Adam Waugh: My mother stayed here for a couple of months 2+ years ago and I decided that I should wait to post a review until after giving it some time. I feel I am in a position to leave an authentic and true review now. The front desk staff was always very kind. Other than that, the staff was demeaning to my mother after her stroke and suggested that she was making things up. I too was worried she was paranoid. After she passed away 6 months after moving out of Cardinal I found a couple instances where she was correct. I gave her my older iPod with some of her favorite music on it. She said that someone took her music; I dismissed this because she had an iPod that looked the same. After going through her stuff I now realize that one of the workers did take her iPod and exchanged it with a similar looking broken iPod. The staff member likely made $20 maximum off of this exchange and my mom had none of her music that kept her happy. This was one of her favorite items. This level of theft is very petty and it is damaging beyond a simple crime. The theft by the staff is present and the ASC staff at multiple levels is aware of it and enables it. I wish I never gave them a chance...but I thought I was in a position to prevent bad things from happening because I could visit everyday. I am not a disgruntled person and I have given myself plenty of time to grieve before making this post. ASC and Cardinal are bad. If you have the option, consider an alternative. They made a difficult time more difficult for our family in need. Find a non-profit care facility if possible. I wish I knew then what I know now.

Darryl Brown: We deliver to them from Jimmy johns but they never tip. They always have some old bat signing for them. If I'm gonna get stiffed on a tip at least let the person who ordered the food do it. Have some balls
Erb N: I've never been to a facility so poorly run and administrated. Communication between the different areas dose not exist to the detriment of the residents. Some of the nurses are not nurses at all and could give you someone else's medication. Beware
sherli anne: I was surprised to reads the above reviews. My sister has been at this home for year. This has been rated one of the best nursing homes and in my opinion deservedly so. I have visited unannounced and found my sister immaculately clean and very well cared for. Her room and the facility was also very clean. I am most impressed by the staff. Her obvious affection for them and theirs for her is apparent. This is not an easy job and I am eternally grateful for the compassion and dedication they show caring for my sister. I also noted they were equally as attentive and aware of the other residents needs as well. Most importantly she is happy there and is very attached to the staff. That speaks volumes. i must admit I can't understand what she is saying, but they do and relay it to me. Another insight into how well they know her and spend time with her.
Autumn Neil: Outdated. Horrible workers. I'm sad my mom has to be here until her Medicaid is re-instated.
David Keenan: One word for you if you are considering this awful place for a family member: don't!
